Enrolling with COPE
To enroll in services at COPE, contact the Enrollment Department at:

The Enrollment Department is open M – F, 8:00am – 5:00pm. If a recording is reached, please leave a message. Intake specialists return calls within ½ hour.

Qualifications for Services
Qualifications for services vary depending on each individual’s needs. Contact the Intake Department for additional information.
We accept AHCCCS, Medicare, and TRICARE, in addition to Private/commercial insurance health plans (See attached list of private insurance companies and publicly funded plans available to members).
COPE also offers a Sliding Fee Scale to all income-eligible uninsured or under‐insured patients. No one will be denied access to services due to inability to pay. There is a discounted/sliding fee schedule available based on family size and income. For additional information call 520.792.3293.
What to Expect at the Intake Appointment
During your first appointment with COPE, an initial assessment will be completed to determine your individual healthcare needs.
What to Bring
The following items are required for scheduled intake appointments:
- State issued ID (driver’s license, passport, etc.)
- Insurance card
- Current list of medications
- Emergency and primary care physician contact information
